Wegovy dose steps and missed doses
Most GLP-1 medicines start low and step up, which doctors call titration. This is what the Wegovy product information says, quoted word for word. UK wording comes from the patient leaflet; US wording from the prescribing information and Medication Guide. Your prescriber’s instructions come first, and GLPMGR never works out or recommends a dose.
United Kingdom
Dose steps, in the product information’s words:
- “When you first start using wegovy®, the starting dose is 0.25 mg once weekly.”
- “Your doctor will instruct you to gradually increase your dose every 4 weeks until you reach the dose of 2.4 mg once weekly.”
- “Week 1–4: 0.25 mg. Week 5–8: 0.5 mg. Week 9–12: 1 mg. Week 13–16: 1.7 mg. From week 17: 2.4 mg.”
- “For weight management in patients with obesity: If needed, a dose increase to the next maintenance dose 7.2 mg once weekly (3 injections of 2.4 mg) can be made after a minimum of 4 weeks on 2.4 mg. The maximum dose is 7.2 mg once weekly.”
If a dose is missed:
- “If you forgot to inject a dose and: it is 5 days or less since you should have used wegovy®, use it as soon as you remember. Then inject your next dose as usual on your scheduled day.”
- “If you forgot to inject a dose and: it is more than 5 days since you should have used wegovy®, skip the missed dose. Then inject your next dose as usual on your next scheduled day.”
- “Do not take a double dose to make up for a forgotten dose.”
Using it with another GLP-1 medicine: “Do not use wegovy® in combination with other medicines that contain GLP-1 receptor agonists (such as liraglutide, dulaglutide, exenatide or lixisenatide).”

Dose steps, in the product information’s words:
- “The recommended starting dosage of WEGOVY injection is 0.25 mg administered subcutaneously once weekly.”
- “Weeks 1 through 4: 0.25 mg. Weeks 5 through 8: 0.5 mg. Weeks 9 through 12: 1 mg. Weeks 13 through 16: 1.7 mg. Week 17 and onward: maintenance dosage.”
- “The maintenance dosage of WEGOVY for weight reduction in adults is either 1.7 mg or 2.4 mg (recommended) injected subcutaneously once weekly.”
- “For patients who tolerate the 2.4 mg dosage for at least 4 weeks and additional weight reduction is clinically indicated, the dosage may be increased to a maximum dosage of 7.2 mg subcutaneously once weekly.”
If a dose is missed:
- “If you miss a dose of WEGOVY and the next scheduled dose is more than 2 days away (48 hours), take the missed dose as soon as possible. If you miss a dose of WEGOVY and the next scheduled dose is less than 2 days away (48 hours), do not administer the dose. Take your next dose on the regularly scheduled day.”
- “If you miss doses of WEGOVY for 2 or more consecutive weeks, contact your healthcare provider to talk about how to restart your treatment.”
Using it with another GLP-1 medicine: “Concomitant use of WEGOVY (semaglutide) tablets or WEGOVY (semaglutide) injection with other semaglutide-containing products or with any other GLP-1 receptor agonist is not recommended.”
Switching between the injection and the tablet:
- “Patients taking WEGOVY 2.4 mg injection for cardiovascular risk reduction or weight reduction in adults may switch to WEGOVY 25 mg tablets.”
- “One week after discontinuing WEGOVY 2.4 mg injection, initiate 25 mg of WEGOVY tablets orally once daily.”
- “The day after discontinuing WEGOVY tablets 25 mg once daily, initiate WEGOVY 2.4 mg subcutaneous injection once weekly.”
